For valuable consideration received, I, hereby grant to Willa Kveta Photography, Inc., also known as Nalaya Healing, and its affiliates, related entities, employees, agents, representatives, successors, and assigns (collectively “Nalaya”), the irrevocable and unrestricted right and permission, in perpetuity, to take, create, use, re-use, publish, and republish photographs, videos, and any other graphical depictions incorporating my likeness in all forms and in all media, in all manners whether now known or hereafter created (the “Content”), without any restriction as to changes or alterations, for advertising, marketing, trade, publications, promotion, exhibition, websites, social media, print sales, book publications or any other lawful purposes. I hereby agree that all rights in and to the Content, including the copyright, are and shall remain the sole property of Nalaya, free and clear from any claims by me or anyone acting on my behalf. I waive any right to inspect or approve the Content, finished version(s) of media incorporating the Content, any text, graphics, or images created in connection with or combined with the Content, or the use to which the Content may be applied, including written copy that may be created and appear in connection therewith. I further acknowledge that I have voluntarily agreed to allow Nalaya to use the Content without the promise of any additional compensation or remuneration in any form whatsoever, and I waive any claim for additional compensation, royalties, or remuneration of any kind for Nalaya’s use or publication of the Content. I agree on behalf of myself (and all my representatives, heirs, executors, administrators, agents, and assigns) to release and discharge Nalaya (and Nalaya’s affiliates, related entities, employees, agents, representatives, successors, and assigns) from any and all liability, claims, and demands of whatever kind arising under law or equity, which arise from or are connected with the creation and use of the Content, including but not limited to copyright infringement, publication of my likeness, invasion of privacy, libel, defamation, false light, or misappropriation of name, likeness, or image. If the nature of the Content includes full or partial nudity, I hereby specifically acknowledge, consent, and agree to appear fully or partially nude in some or all of the Content. I warrant that I am of full legal age and have the right to contract in my own name. I have read the above Model Release prior to signing. I understand the legal consequences of its contents, meaning, and impact, and I freely accept the terms. This release shall be binding upon me and my heirs, legal representatives, and assigns. By signing this Model Release, I represent that I have read and understood this document and I agree to be bound by its terms.

Assumption of Risk and Liability Waiver: I understand the physical risks associated with my participation in this experience. I agree on behalf of myself (and all my representatives, heirs, executors, administrators, agents, and assigns) to release and discharge Willa Kveta Photography and Nalaya Healing (and WKP’s affiliates, related entities, employees, agents, representatives, successors, and assigns) from any and all liability, claims, and demands of whatever kind, arising under law or equity, which arise or may hereafter arise from or are connected with my participation in this Session.
